Bugfix release for holo-codex@0.1.2.

Install:

npm install --global holo-codex

Runtime identifiers remain stable: agent-loop, .agent-loop/, and autonomous-pr-loop.

Note: this release was published from the maintainer local npm session while npm Trusted Publisher setup remains tracked in #24.
