Holding up at fleet scale. The incident window answers in milliseconds on a 10,000-host estate, every MCP answer states its own scope and freshness, operator annotations are shared across replicas, and three fixed limits become configuration. Additive, no data migration.

- Change reads skip what they exclude (#352). Measured on a 311,750-entity estate over a window of 380,000 heartbeats:
