Ship CLI/MCP reconcile with mocked tests, doctor track_o_debt, and execute-lane skill/workflow docs so selected → pr_opened → terminal no longer depends on manual memory.
Scan all outcomes for Track O debt and terminal idempotency, and include outcome reconcile guidance whenever track_o_debt coexists with other data_store warnings.
…ures. Doctor now warns when Track O debt scanning throws, and MCP/CLI reconcile refuses write=true together with dry_run=true.
Drop weak body-superseded auto-writes, skip GitHub author lookup when debt is empty, and copy acted_against_skip from open-lane rows onto terminal events.
