Added

• Native banners for tool-permission prompts ("Allow reading from X?"). The VS Code extension fires no hook for these, so a waiting-detector notices when a Read/Edit/Write tool is left blocked on your approval and raises a banner.

Fixed

• Click-to-focus opened a new VS Code window when the session's working directory was a subfolder of the project. The click now resolves to the project root and focuses the existing window (also fixes focus-aware suppression and notification grouping for nested directories).

• Renamed the project to claude-banners-macos (was claude-toast-macos) — now a standalone macOS tool.
