conversational-output.md — new NEVER rule bans #/## heading levels in conversational replies; headings must start at ### to save presentation space. Updated the matching ALWAYS rule, conditional table, and multi-topic example to reference ###+ instead of ##/###. Promoted live → bundle (skills/init-project/portable/rules/conversational-output.md).
