Fix History visual diffs losing valid changed regions inside nested document containers. Use the compiler-backed source range mapping directly; enclosing-element caret positions no longer discard paragraph changes.

Verified the exact Cherry/Hanami wording edit with native Tinymist and production browser/WASM, including screenshots. Comment-only changes still show Source Diff.
