docs: add per-crate READMEs and detailed install guide for the 3 CLIs#7

Write a README for each workspace crate (hoangsa-cli, hoangsa-memory, hoangsa-memory-mcp, and the six memory library crates) so each surface is self-documenting. Expand the root README install section with platform support, release-installer flags, build-from-source paths, per-CLI post-install steps for hoangsa-cli / hoangsa-memory / hsp, uninstall, and a troubleshooting table — so users can get from zero to working toolchain without digging through scripts/install.sh.
