Add Usage Insights with local metadata-only usage metrics for dictated words, sessions, estimated time saved, daily trends, and top apps.

Replace the global cleanup toggle with app-specific Cleanup Groups, including a default group for unassigned apps and per-group cleanup mode/provider/model/prompt settings.

Add recently used apps from usage metrics to cleanup group app assignment, alongside running apps and manual app selection.
