Fixes

• fix(harness/opencode): Default to fully autonomous permissions when bridge spawns opencode. The previous permission block only covered edit/bash/webfetch and only when permission_mode was set, leaving opencode's ask defaults for external_directory / doom_loop and the hardcoded .env read carve-out in place — any of which would block a headless run. The new block emits an explicit granular allow-everything permission map, including overrides for **/.env and **/.env.* on both read and edit.
