fix: count only real prompts with text - #96

Claude Code writes empty user entries for tool permissions and auto-responses. Now only counts user messages with actual text content. Before: 83 "prompts" today. After: 10 real prompts. Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>
