v11.2.0 — per-representation staleness + drift detection

Wave A: per-repr half-life decay, edge freshness in context_expander, recall noise filter (recovery/auto-extract).

Wave B: parent_content_hash drift
detection + re-enqueue on memory_update, per-tier decay folded into RRF via score_weight callback.

Migration: 027_repr_freshness.sql adds parent_content_hash + last_confirmed columns.

Tests: +38 new across 5 files, full
suite 1759 passed.
