upgrade v0.42.1 + derive cwd from plugin api

DEPRACTION:
- `ghost_launcher` config
- `exec_cwd` config

Now the cwd for the RunCommand pane is derived from
`get_plugin_ids().initial_cwd` provided by the plugin API.
