v1.10.98 — server/harness fixes

  • #807: server conversation dedup on load — self-heals duplicated history (fixes oversized context / long-context errors from re-appended messages).
  • #804: Monitor background-task pull-model — task_list / task_output / task_stop tools + Background Tasks UI panel; output reuses the existing tool-result store.
  • #801: tightened harness->LLM messages (concise, fact-first; loop/todo/read-coverage/stash notices).
  • #802/#803: stash threshold raised to 100KB (no re-fetch of small reads) + Monitor notification coalescing.
  • #797: adversarial verify-gate content ported (library; not yet wired).
