feat(eve): persist instrumentation provider state

Summary

Related to #1659 and the proposal in #1799. Stacked on #1846 and still gated by experimental.instrumentationProviders.

Provider-local maps cannot carry correlation data through serialized workflow steps or replacement workers. Handlers now receive JSON-valued state scoped by provider slot and event idempotency key; starts can pass state to terminals, terminal and attempt cleanup releases it, and cancellation preserves it long enough for the cancellation epilogue to close outstanding operations.

State leases are revoked when a handler returns, preventing late reads or writes. Values are serialized into the durable context, so provider authors remain responsible for keeping them small.

Validation

• Full rebased eve unit suite: 6,163 passed, 1 skipped
• Full eve integration suite: 618 passed
• Focused provider, eval, tracing, and compiled-artifact scenarios: 14 passed
• tsc -p packages/eve/tsconfig.json --noEmit
• pnpm guard:invariants; docs checks pass
• E2E not run locally; the provider layout remains behind the experimental flag
