feat(eve): sources - derive modules from selected slots

Summary

Related to #2347. Programmatic sources could only contribute fixed, independent modules, which left features such as memory to implement ad hoc projection and loading behavior. This adds a generic derived-source primitive: a selected direct module can instantiate a registry-issued template in another ordinary slot with explicit dependency aliases and serialized parameters.

The causal anchor now determines the derived candidate's node and precedence layer, and eve derives its stable source ID from the template, target path, and anchor. Every dependency must belong to the anchor's node, the anchor must be present among them, and only templates returned by the registry can be instantiated. Direct candidates win over derived candidates within the same layer. Generated, in-memory, and hydrated module maps all resolve the same dependency graph and cache loaded namespaces once per phase.

This is a precursor to #2534. That PR will rebase onto this primitive and remove its memory-specific source-graph machinery. Shared provider-instance materialization remains intentionally out of scope here and must be solved generically during that rebase.

Validation

Focused unit coverage verifies registry-issued template handles, anchor inheritance, stable identity, cross-node and shadowed-dependency rejection, same-layer precedence, dependency ordering, namespace caching, generated ordering, and malformed dependency graphs. A non-memory proof selects a connection that induces a tool across extension and application layers, then verifies that a direct application tool wins.
