Example Bug Report

Name/Description: Header bar - Logout button does not log user out when clicked.

Location of defect: Header bar

Steps to Reproduce (required):

1. Open application.
2. Log in to the application as an administrator.
3. Click the logout button in the header.

Expected Result: Take the user back to the login screen.

Actual Result: The logout button does not do anything when clicked.

Dataset: Administrator user role / Default

User: Administrator / N/A

Configuration notes: Only observed on Windows 10 Firefox.

Workaround: A way for the user to get around the defect and continue their work.

Severity:

• Low: Visual defects that are easily overcome by the user and do not affect their data or their workflow.
• Medium: Defects that may affect the data or workflow, but have clear workarounds.
• High: Defect that may greatly impact data or the user's workflow and their are no clear workarounds.

Priority (Response Times):

• High: Might need to be fixed before the next release
• Medium: Can be fixed within any of the next 6 months of releases
• Low: Have no set time that they need to be fixed by
