🎉 refactor!: migrate to v2.0 config structure with map-based format

BREAKING CHANGE: Config file format completely redesigned

Old format (v1.x):

  mcp_servers:
    - name: "server"
      clients: {claude_code: true}

New format (v2.x):

  mcpServers:
    server: {...}
  clients:
    claude_code:
      enabled: [server]

Changes:

• Refactor config from array to map-based structure
• Move server names from fields to keys
• Move client enablement from per-server to per-client
• Fix field preservation bug (type, httpUrl now work)
• Add test dependency to build/release tasks
• Update all documentation and examples
• Simplify code (-880 lines)

Migration required: Users must manually convert config files
