Fixed

Code blocks with # comments: a # line inside a fenced code block (Python, shell, YAML, Ruby) no longer renders as a heading and splits the box in two.
Stray glyphs when breaking or completing a code fence (a closing fence sometimes drew as garbage) are fixed.
Reading mode no longer draws image resize handles.
A broken or unavailable image now shows a compact placeholder instead of leaving a large empty gap.

Changed

Settings > Appearance: the app icon chooser (White / Dark) now sits under the Mode picker, laid out to match the color rows.
