• Fix claude agent_usage_recorded model field (was always
  "unknown"; now extracts from claude's assistant event).
• Fix claude agent_usage_recorded input_tokens field (0.1.24
  simplify introduced wrong NET formula assuming Anthropic schema is
  gross; it's already net). Aggregation scripts reading 0.1.24 data
  will see non-zero values now — this is the correction, not a spike.
• Fix gemini preset: -o text → -o stream-json so the
  gemini_error_detector plugin can parse output. Existing scaffolds
  need manual TOML edit; round log format changes (text → JSONL).
