v0.32.1 — review fixes for 0.32.0

Six findings from reviewing 0.32.0 — all of them a promise written on one side of a contract and
not on the other.

Fixed

• review-data-integrity now reads the Read Model / CQRS / Event Sourcing table and reports an
  absent aggregate as undecided, as design-data-layer already claimed it did.
• map-domains writes the Domain Vision Statement's VIS- into the upstream of every Core
CTX-, so the edge adapt-change and /product:review rely on actually exists in the graph;
  /product:review flags a Core with no VIS- upstream.
• /product:review checks the Bounded Context Canvas (all nine parts; a CTX- redrawn on a rerun
  is a recorded decision), as the Canvas section said it did.
• design-scalardb / design-data-layer list reports/04_quality/nfr.md as an optional
  prerequisite with the pure-architect fallback stated, instead of naming it only in the body.
• The DDD-coverage artifact check counts only SKILL.md, manifests and the output tree as
  declarations — a rule discussing a path no longer makes it "declared".
• generate-test-specs no longer lists generate-scalardb-code twice in Related Skills.
