Gaps the first end-to-end runs of /architect:design-api (AsyncAPI from the catalog) and
/architect:redesign (opening the ADR log from scratch) surfaced.

Fixed

• design-api and the catalog. An orphan event that already has a channel is kept and
  reported, never dropped (a §8 breaking change); constraints are the contract's own, the catalog
  gives names; a field the catalog lacks stays deprecated and is removed per contract-fidelity
  §8; idempotency-key ownership is one-directional — the value is the catalog's, the dedup rule is
  the contract's; cross-reference findings in another skill's files are reported, not blocking;
  grpc/ is emitted only when adopted; the canonical validator is named.
• The ADR rule, again. decided_at is when the decision was taken; an answered OQ- is a
  valid driver; anchors are free text and unresolved; the index carries standard frontmatter with
  the last regenerator as skill; a record's frontmatter is the rule's shape, stated as the one
  exception in rules/output-conventions.md; numbers are never re-minted; the four appending
  skills defer the upstream instruction to the rule instead of restating it wrongly.
• redesign. The Canvas Name falls back to BC-n on the legacy path; aggregate boundaries
  are design-aggregate's decision and get no ADR from redesign.
