- fix: rescan after coming back from death
While dead or a ghost no item is usable, so a scan then wrote the empty
macros and nothing rescanned on resurrection. Scans now wait while dead,
like in combat and on a taxi, and run a second after PLAYER_ALIVE or
PLAYER_UNGHOST.
