On Record is a prototype for consented, de-identified community requests with independently verifiable provenance. It is designed around a practical problem: a person may need to return to a request without a reliable device, email address, username, or password. It is not an HMIS, coordinated-entry system, shelter-placement service, emergency line, medical record, or live bed dashboard.

The prototype keeps a request, its consent record, its original wording, its disclosed transformation, and its integrity proof together. A signature can show that bytes changed; it cannot show that a story is true, that consent was freely given, that a provider is authorized, or that a service is available.
Never put a real person’s name, date of birth, ZIP code, health information, recovery credential, immigration information, victim-service information, or precise sleeping location into the demo or a public discussion. Use composite examples only. Call 911 for immediate danger, 988 for a mental-health or suicide crisis, and 2-1-1 San Diego for current local service referrals.
