Recovery and documents

This toolkit is designed for paper, borrowed phones, kiosks, and advocate support. Keep it private. Never place document numbers, recovery codes, DOB, ZIP, or health details in a public record.

Returning to a request

Ask an advocate to read back the record, show what is public, and issue a paper recovery card. Keep the card separate from public materials. If it is lost, request an in-person restore; do not hand a code to someone who promises help. Ask for a correction or withdrawal whenever needed.

Replacing documents

Start with a private inventory and a safe mail destination. Ask about fee waivers, homeless-status affidavits, transportation, translation, disability accommodations, and General Delivery before travel. Use official offices or 2-1-1 to confirm current requirements for birth certificates, California ID, Social Security cards, and DD-214/veterans records. Track submissions and follow-ups privately.

Emergency health card

The printable card is optional and paper-only. Fill only what you want responders to know; do not upload, photograph, or post it. Keep old copies secure and destroy them when outdated. Medical staff may still need to confirm information.
