Tool surface overhaul: consolidated 69 tools to 45, every tool brought to full definition quality. Progressive tool disclosure is now the default to cut token usage. Breaking: tools renamed/merged (search_content to search, update_metadata to update_document, recover_document to restore_document). Fixed semantic_search registration and parallel test races.
