What's new

Added file-scoped Click Git commands to the editor body right-click menu.
Updated command fallback so file commands invoked without an explicit URI operate on the active editor file, not its parent folder.
Updated README media to show four entry points: Explorer folder, Explorer file, editor tab, and editor body.

Full local harness passed: lint, compile, typecheck, unit tests, real Git fixture tests, VS Code extension host tests, and VSIX packaging.
VS Code integration coverage now includes active-editor file staging without an explicit URI.
