- ✅ Grafana failed to self-update bundled plugins on every boot: Grafana 13's preinstall_auto_update default tried to update the unused bundled elasticsearch/zipkin plugins each start and failed under cap_drop ALL, logging two ERROR entries per boot; disabled via GF_PLUGINS_PREINSTALL_AUTO_UPDATE=false so plugin versions stay pinned to the image. Verified live: zero error-level grafana log lines on a fresh boot (#1944).
- ✅ pgAdmin logged a blank-user servers.json re-import error on every restart: the native entrypoint's re-import needs PGADMIN_DEFAULT_EMAIL, which the restart path never loads, producing a confirmed no-op with misleading errors; the restart path now sets PGADMIN_REPLACE_SERVERS_ON_STARTUP=False, skipping the import and preserving user edits per the entrypoint's documented contract. Verified live across a restart with the server list intact (#1945).
- ✅ OVERRIDE-prefixed issue titles failed validation: AGENTS.md Section 8 mandates the [OVERRIDE] prefix for emergency-override issues, but create-issue.sh and check-pr-ready.sh rejected it; both now accept an optional, narrowly scoped [OVERRIDE] prefix before the type tag (#1947).
