Fix live-updating commands creating infinite new lines

Remove the [AI Ready] banner that was injected directly into xterm.js,
bypassing ConPTY. This caused a persistent 2-line buffer desync between
ConPTY and xterm.js, breaking cursor-up sequences used by programs like
lintrunner for in-place display updates.

Also add remote session (SSH/Telnet/Serial) support for AI, spawning the
CLI process locally instead of injecting shell commands.
