Improvements

• Simplified the PTY session runtime boundary and removed stale tmux-era paths.
• Consolidated per-session PTY state so live handles, retained output, alt-screen state, resume state, and kill state are tracked together.
• Split the session manager into focused modules for spawn, lifecycle, output, and tests to make future agent and human investigation easier.

Bug Fixes

• Tightened Codex rollout output capture.
