Fix: Segment info on code changes

Added source_segment and source_video to code_changes items in both schemas
Post-compilation fallback in normalizeAnalysis inherits segment info from parent ticket to nested code_changes, comments, and video_segments when Gemini drops them
Code changes now display segment labels (e.g. Seg 1) instead of bare timestamps
