dsh-rule-evolve v0.6.0 — rule lifecycle

Rules now have health scores and the library self-maintains: score (active/stale), prune (soft-retire below threshold, auditable), merge-duplicates (near-duplicates merge into higher-scoring rule). Dogfood: 5/5 active, no duplicates. 11/11 tests.
