Plugin gains evolve_recall: agents retrieve the most relevant verified rules mid-session (BM25-lite, offline). Fixed a real BM25 bug found by the new test (document frequency double-count). CLI 12/12 + plugin 6/6 + packed runtime smoke (4 tools).
