Fix same-task worker resume after a recoverable handoff-uninspected denial, bind resume authorization to the original child session, and preserve fail-closed lifecycle cleanup.

Validation: npm test (316/316 PASS); WSL OpenCode CLI same-child Read then bind PASS.
