Improvements

• Custom cover images for folders, authors, series, genres, and collections (#70, #20). Long-press any folder or stack and tap 'Set ... image' to pick a file, or drop a cover.jpg into a folder and Bookshelf picks it up automatically. For stacks, you can also drop named files into a hidden image library at /.bookshelf-images/{authors,series,genres,collections}/ for batch setup. README has the details.

Bug fixes

• Collections chip now paginates correctly past the first page of 8.
