Bug Fixes

• ensure blank lines are emitted after nested lists if necessary
  Previously, the input:

  1. item
  
     * a
     * b
  
     para
  

  would roundtrip as:

  1. item
  
     * a
     * b
     para
  

  This is incorrect, because the CommonMark spec would treat para as part of
  the b item.

  Fix this by tracking the last event, and using that to emit a blank line if
  necessary. This isn't perfect but it should handle the vast majority of cases.
