v0.1.3: wire Azure Artifact Signing for Windows installer

CI installs AzureSignTool on the Windows runner and signs nsis +
portable .exe via build/sign.js. Custom signer is a no-op when
AZURE_CERT_PROFILE_NAME is unset, so unsigned builds still work
locally and in forks without secrets.
