… portable creds, dispatch, shell_integration tests (#47/#48/#41/#35/#7/#50) (#936)

Six macOS review fixes, all code-complete and unit-tested on this Windows dev
box (no Mac hardware needed for the code or tests):

- #47: braille pack download gated Windows-only via braille_install_supported();
  macOS shows a Homebrew liblouis hint (is_braille_pack_installed already
  detects a Homebrew install). The pinned pack ships lou_translate.exe.
- #48: external_engine._resolve_which now checks macOS well-known dirs
  (/usr/local/bin, /opt/homebrew/bin, /opt/local/bin) and the highest nvm
  version under ~/.nvm/versions/node, so a Finder-launched app (minimal PATH)
  finds a real Homebrew/nvm Node install.
- #41: the three LibreOffice call sites resolve soffice via the new
  external_tools.libreoffice_executable(), which finds the standard macOS
  /Applications/LibreOffice.app/Contents/MacOS/soffice (never on PATH) instead
  of the bare "soffice" argv that missed every standard Mac.
- #35: portable mode no longer silently drops credentials on macOS; it routes
  load/save/delete to the login Keychain (best available store) with a
  warning, via extracted _macos_keychain_* helpers shared with the non-portable
  path. No DPAPI-equivalent single-folder store exists off Windows.
- #7: quill.platform.dispatch delegates to the module-level-gated root helpers
  (high_contrast, sr_detect) instead of carrying its own copy of the platform
  branch, so a future macOS-routing change applied once is picked up
  everywhere.
- #50: the macOS shell_integration module (Info.plist doc types, the duti
  association path, off-darwin/missing-duti no-op branches) now has direct
  test coverage.

Also moves the three "needs real hardware / fresh repro" macOS items (NSSound
audibility, file-open/chord confirmation, intellisense dead-popup latent risk)
from docs/planning/review.md into the beta-2 release notes as a "help wanted
from macOS users" entry, and trims the resolved items from review.md (#3/#7/
#35/#41/#47/#48/#50). Deferred this batch: #10 (relocate platform-neutral
sr_announce/announce_engine — pure refactor, many import sites) and #13 (add a
platform field to gate scan schemas — broad snapshot reshape).

GATE-11 rebaselines: structured.py 702->712, main_frame_speech.py 2879->2894.
Docs (PRD, user guide, CHANGELOG) + HTML/EPUB artifacts regenerated.
