O on an empty cell repeats the last chain, phrase or instrument; O twice makes a new one.
On the phrase screen the new instrument is a copy of the one you were using, in the next empty slot.
The notice line says whether you got a repeat or a new one.
Insert and delete song rows on TRIANGLE: up or down inserts, X deletes after a yes, both undo with R+L.
