feat(rules): redesign magic and ritual systems

Introduce "The Toll of Magic" mechanics, including Witch-Marks and
Backlash effects. Update the spell and ritual tables with new visceral
effects, conditions, and requirements to align with the darker thematic
direction of the setting.
