Add CMS ACA marketplace source packages

Summary

• adds CMS ACA 2024 OEP state-level source-package facts for marketplace enrollment, APTC recipients, and average monthly APTC
• adds CMS ACA 2022 OEP state-level facts and CMS 2022 effectuated enrollment / financial-assistance facts
• wires source-package aliases for the new CMS ACA packages and covers counts/sample facts in focused tests
• maps CMS 2022 total enrollment to the canonical cms_aca.marketplace_effectuated_enrollment concept so Microplex can compose CMS base enrollment with KFF 2024 target enrollment

Microplex check

Built these packages plus kff-marketplace-effectuated-enrollment and loaded them through Microplex's ACA PTC multiplier helper. That produced 51 state multiplier rows; California uses the OEP average-APTC base and Nevada uses the effectuated-enrollment fallback.
