Real vector semantic search is here. ctx_semantic_search now performs true nearest-neighbor search over the code-index database instead of always falling back to keyword search.

Features

• Vector semantic search: ctx_semantic_search embeds the query via Ollama, loads vectors joined with chunk metadata, and returns similarity-ranked code matches.
• repo_id scoping: new optional repo_id parameter scopes vector search to a single indexed repo.
• Real embeddings on index: LocalIndexer switched from the ZeroEmbeddingProvider stub to OllamaEmbeddingProvider, so indexed repos store meaningful vectors (gracefully falls back to zero vectors when the proxy is unavailable).
• Graceful degradation: keyword-search fallback preserved when embeddings are unavailable or none are indexed yet.

Internal

• New list_vectors_with_chunks SQL join (vectors + chunks metadata) with unit-test coverage.
• LocalSearcher::conn() accessor for read-only callers that join the vectors table directly.

Chores

• Workspace version bump 1.4.0 -> 1.4.1 across Cargo.toml, Cargo.lock, and plugin manifests.

Fix: use forward slashes in Windows hook paths to prevent bash escape-sequence misinterpretation. Legacy file-based hooks no longer installed for Claude Code — only declarative settings.json hooks.
