Fix Windows hook execution with Git Bash:

  • Windows hooks now use context-mode.exe directly instead of context-mode.cmd
  • Git Bash cannot execute .cmd files, but can run .exe binaries directly
  • Eliminates: /bin/sh: cannot execute binary file and context-mode: command not found
  • settings.json hooks and ~/.claude/hooks/*.cmd scripts both fixed
