Account Entry Missing Phone Number Labels on Mobile

Description

The Phone Number Type/Control Labels get hidden when using Account Entry block on a device < 768px wide so it isn't possible to tell what number you are entering it for. It also is not just done in CSS so it can't be overridden easily. This is a change after version 16.13.

Actual Behavior

The Account Entry Block hides the phone number type/label on mobile devices. 17+

Expected Behavior

The Account Entry block doesn't hide the phone number type/label on mobile devices. As it worked in Rock 16.13 and lower.

Steps to Reproduce

1. Configure your "Account Entry" block (such as on page id 208 or /NewAccount) to Show Phone Numbers and choose more than one Phone Type to display (I chose Mobile, Home and Work)
2. View on mobile device or browser window < 768px. You should see the phone type label of "Mobile", "Home" or "Work" disappear and it is unclear which number you are entering.
