Improvements

Introduces a new boot menu (ported straight from the current Apple // version).
Now supports flexible volume configuration: volume 1+2 can now be selected independently of SD card 1+2. However, mapping volume 1 to SD card 2 (or volume 2 to SD card 1) only works with newer firmware of the DAN][ controller (firmware >= 3.0.0).
Boot menu now supports "interactive configuration" using the arrow keys.
Boot menu uses colored text mode on the Apple /// (or monochrome green shades :) ). Note: colored text mode has no 'flashing' feature, only inverse text and colors (shades).
Boot menu supports configuration of the IP address (for the optional WIZnet module). Press 'I' to configure the IP address for the FTP server.
