Codex Beacon 0.9.2 — Fix clearing read task history

修复“清空已读”后任务变成“状态未确认”的问题。

• 已读历史按完整任务轮次删除，完成提醒与较早的运行记录一起清理。
• 进行中、等待回答、等待审批和未读任务保留完整状态依据。
• 已读且未确认仍在运行的历史任务可清理；删除后日志重放不会使任务重新出现。
• 主扩展更新为 0.9.2，采集器保持 0.5.10。任务结束后重新加载 VS Code 窗口使修复生效。

Fix task states becoming “Unconfirmed” after clearing read history.

• Remove read history as complete rounds, including older running snapshots.
• Preserve all state evidence for confirmed live tasks and unread reminders.
• Allow clearing read unconfirmed history; replay cannot restore deleted rounds.
• Main extension 0.9.2; collector remains 0.5.10. Reload VS Code when current work has finished.
