Codex Beacon 0.9.3 — Distinguish approval review from blocked tasks

修复自动审批被显示为“阻塞”的问题。

• Codex PermissionRequest 在自动审核前触发，不能证明任务正在等用户手动批准。现在显示紫色“审批处理中”。
• 任务详情说明该状态的含义，通知改为“Codex 有审批请求”；审批请求仍可从仪表盘、进行中和等待处理筛选中访问。
• 保留明确的等待回答／等待审批；匹配的 PostToolUse、模型继续执行或轮次结束会解除审批请求，Token 更新和无关并行工具结果不会解除。
• 仅凭当前 Hook 无法区分自动审核与手动等待，请在 Codex 中查看是否需要处理。
• 主扩展 0.9.3，采集器 0.5.11；任务空闲后重新加载窗口生效。

Fix automatic approval review being shown as a blocked task.

• PermissionRequest precedes automatic review and does not prove manual approval is required. Show purple approval processing instead of a manual block.
• Dashboard, sidebar, filters, task details and notifications distinguish approval requests from explicitly observed waiting states.
• Keep verified PostToolUse matching and model-continuation recovery, while preserving real waits across token updates and unrelated parallel results.
• The hook alone cannot tell whether Codex is waiting for manual approval. Open Codex to check whether action is needed.
• Main extension 0.9.3; collector 0.5.11. Reload when current tasks are idle.
