This PR fixes two critical issues in the session manager: migration race conditions and missing message_id in conversation forks.

- Wraps all database migrations in a transaction to prevent corruption when multiple goose instances run simultaneously
- Fixes message insertion during conversation fork to include the required message_id field

nice catch - and I did see comments in the past about outside of tx so this explains why, never thought about the failure mode of multiple things hitting sqlite - amazing how well it holds up |
