Docs-only release: documents the RTL (right-to-left) support status of VS Code's new WYSIWYG Markdown Editor and adds workaround guidance. No changes to extension behavior.

• README: new Right-to-left (RTL) text in VS Code's Markdown editors section covering what each VS Code Markdown surface does for RTL text, plus workarounds (text editor, preview, <div dir="rtl">, markdown.styles).
• Landing page: new RTL section at https://noote.iamsajaad.com/#rtl.
• CHANGELOG: backfilled entries for 0.0.4 and 0.0.5.
• Packaging: added .kilo/** to .vscodeignore so plan files can never ship in the .vsix.

RTL tracking

VS Code's new Markdown Editor (vscode.markdown.editor) has no RTL support yet, and extensions cannot patch its sealed webview.
