First release of the CostKits MCP server.

• 12 tools mapping 1:1 to the CostKits REST API: estimates, patient liability, providers, coverage rules, procedure ontology, bill analysis, plus a no-auth demo_estimate
• Surfaces the API's agent_hint error field verbatim for self-correcting agent loops
• Smoke-tested through the real MCP stdio path against the live API
