v0.1.0 — watch them work

First release. dsh could already delegate to Claude Code and Codex — one
task in, one sentence out. It could not let you watch.

Each crew member now gets a real PTY running its own CLI in the dsh
session's workspace, streamed to an xterm pane in a Crew tab beside
Chat and Trajectory. You can type into any pane at any moment. Five tools
— crew_list, crew_seat, crew_send, crew_peek, crew_dismiss —
let the dsh agent run the team while you watch the same terminal.
